Call or text, email, or submit the form below. Tell us the evaluation type needed (Full Body or Upper Extremity), the case type (workers’ comp, disability, return-to-work, or litigation), and the best contact for scheduling — and we’ll take it from there.

Not sure which FCE to request?
Choose a Full Body FCE for general return-to-work and whole-body capacity questions, or an Upper Extremity FCE for hand, wrist, elbow, and shoulder claims. If you’re unsure, select “Not sure” on the form and we’ll help you decide. You can also review the full FCE overview.
